  • School Health Index
    The CDC designed this tool to allow parents, teachers, and school officials to assess the strengths and weaknesses of health promotion policies and programs as they relate to nutrition, physical activity, and tobacco use. The tool has Score Cards you can use for Elementary Schools or Middle/High Schools. After completing the assessment of your child's school, you can use this tool to develop an action plan for improving school health. There are also hyperlinks to many useful resources for school health and safety.
